The Monstrous will spawn in the exact spot one of the Ancients was killed - like a rare ore node it spawns INSTEAD of the standard one (not like Golgannar or Bufo who spawn on their own timers, regardless of how many rock monsters or frogs you kill). In otherwords, killing ancients is the only way to trigger the spawn of the Monstrous.
3. I found that by doing the southern route I was able to generate approximately 1-2 chances per hour of grinding. Sometimes I got zero, sometimes 3 or 4.
